On this page you will find a list of our fees, forms to download, and insurance info. The fees listed are for self-pay clients but if you have insurance your fees will be based on your plan. With many plans you would pay the specialist copay. There is list of insurance plans accepted by our therapists. Please check to see which therapists accept reimbursement by your plan and bring your insurance card to your first appt. There are intake, disclosure and information forms available for you to download, fill out, sign and bring to your first appointment. 

You can pay with cash, check, Visa, Mastercard, Discover and Debit. Unfortunately we do not accept American Express. You have the option of using your HSA card as well. Thank you again and don't hesitate to give our experienced admin staff a call if you need help.
